School personnel shall give prescription medication to students only when a written request from the physician or nurse practitioner is received detailing the name of the drug, dosage, time to be given, and possible side effects and with written parent permission. Parents must deliver the prescribed medication and necessary equipment with which to administer it, in person to the school in its original container. (ex. syringes for insulin administration)
2. If [over the counter (OTC)] non-prescription medication is required, it must be supplied to the school in the original container by the parent. Written parent permission must be provided with specific directions for administration.

Permission to self-administer non-prescription medication may be revoked if the student violates this policy and the student may be subject to disciplinary action in accordance with the Standards of Student Conduct.
