NCAA Clearinghouse Request Form

Students planning to participate in collegiate athletics and/or to participate in the collegiate athletic recruitment process should register with the NCAA Eligibility Center at the beginning of their Junior year. To register, you will need to do the following:

 

• Register at: NCAA Clearinghouse

• Be prepared to pay the NCAA Eligibility registration fee by Visa or Mastercard ($60 before September 1, 2010; $65 after September 1, 2010)
  

• Print & complete the Transcript Release Authorization 1 & Transcript Release Authorization 2 from the NCAA Clearinghouse site.
  

• Complete the HVHS NCAA Clearinghouse Request Form (also available in the Counseling lobby)

• Complete a RCPS Authorization for Release of Information (also available in the Counseling lobby)

• Turn all completed documents into the Counseling Department as soon as possible to be processed.

Release of Information

Request for General Transcript (this document should NOT be utilized for college applications)

• This document should only be used by CURRENT HVHS students or students who graduated last school year. All other previous HVHS student records are housed at Roanoke County Public Schools' Central Office and must be requested directly from Central Office.

Work Permits (click here to view information regarding child labor in Virginia)

• For your convenience, work permits are available in the counseling lobby. Work permits are issued to 14 & 15 year olds. When all of the documentation indicated below have been completed, please bring all items to one of the following locations: Hidden Valley High, Cave Spring High, Glenvar High, Northside High, William Byrd High, or Roanoke County Schools Administration Office. To obtain a work permit, you must:

  •  Complete the small white form in the work permit packet (Must be completed by the employer)

  • Complete the small pink form int he work permit packet (Must be signed by the parent - either notarized or signed in the presence of the issuing officer)

  • Bring a birth certificate when returning the completed forms to the issuing officer. (A bible baptismal record, insurance policy at least one year old, passport, school record, or driver's license will be accepted in lue of a birth certificate if one is not available.)
